Case Managers (CMs) are responsible for coordinating all aspects of care for the beneficiary & ensuring access to both waiver and non-waiver supports.

You must meet the following EPD Waiver program eligibility requirements:
- Be a District of Columbia resident.
- Be a U.S. citizen or qualified resident.
- Require assistance with activities of daily living (such as dressing, using the bathroom, and grooming)
- Be elderly (65 years of age or older), or 8 to 64 years of age and diagnosed by as physician as having a physical disability.
- Meet a nursing home level of care (LOC) Liberty Assessor
- Be Medicaid eligible and maintain your Medicaid eligibility.
